If your confused then sort it out yourself why do you need help from others? 
Therapy is just good for the brain. Many people get it from others in their life but some people either don't have anyone to fill that gap or feel that there's no one so they go to a therapist
or let it boil inside until they eventually snap, murdering everyone they once loved.
Therapy is just good for you even if nothing is wrong. I've heard several people mention they weren't having any "issues" but they just went to one for the hell of it and said they just feel better afterward. So, naturally, if someone is confused with one really big aspect of their life, it would make sense for them to see a therapist about that specifically.
If you're wondering about the existence of such a specialist, I mean any medical/psychological/neurological/etc. field is going to have specifications. I suppose the more weird and obscure your specification, the easier it will be to make money due to a lack of competition.
But they do. And someone has to prescribe pills and help them with the mental process of taking in estrogen.
